To request a court report, please print and fax the form below.

**Please note that there must be an appropriate release of information signed by the client and/or guardian in order to complete and release information in a court report that will report on client progress.  The client and/or guardian is to specify in the release what information is approved to be released.  All releases will follow guidelines of HIPAA compliance.
